What is an affidavit of birth?
An affidavit of birth is a sworn legal document that a person makes to confirm the other person’s details of birth. This template can become helpful in various situations, including:
- when information reflected in the original birth certificate is wrong or incomplete;
- when the birth certificate was lost or unavailable;
- when immigration or other state authorities require an alternative form of proof for the provided person’s birth details.
