Both parents have to sign if:
– a child lives together with both parents;
– if parents are divorced, but they hold a parental responsibility over a child.
One parent or legal guardian has to sign if:
– one parent has a parental responsibility over a child (for example, by virtue of a court order);
– a child has only one living parent or guardian (for example, another parent died).
